foreign team policies and procedures

It is NOT necessary to submit both applications. Please read the information carefully to determine which application is best for your organization.
2024-2025 Club Application
Click here to access the 2024-2025 Club Application. If you wish to register your club with the Florida Region of USAV, please complete this application prior to completing the event sanction form. If you do NOT wish to register your club, please submit the Affiliate Application below. More information about club requirements can be found HERE.

step 3 - submit an event sanction application

Click here to view the Event Sanctioning Guide. We have implemented a tiered event system to give tournament and event directors more freedom to deliver an event that suits their target market. Each sanction level will have specific benefits and requirements.

Event sanction application
Click here to access the 2024-2025 Event Sanction Application. Use this online form to request the approval of a future USAV sanctioned activity or event. Please submit an Event Sanction Application for each event (tournament, scrimmage, clinic, etc.) you are hosting. If you are running a league, please submit one Application for each section of the league (i.e., Summer Beach League, Fall Indoor League, etc.).
Please do not submit this application until your Club Application or Affiliate application has been approved.

step 4 - request a certificate of insurance

Click here to access the 2024-2025 COI Request Form. Use this online form to request a certificate of insurance for a facility being used to host a USAV sanctioned activity. Please submit a request for each USAV sanctioned event or activity.
