Event Sanction Process

Step 1 - review the following information about event policies
All Florida Region of USA Volleyball sanctioned events are required to follow these policies and procedures. Please review the following information carefully.
  • Tournament Director & Official's Director Policy - This policy outlines the specific requirements of TDs and ODs when operating a USA Volleyball sanctioned event in the Florida Region.
  • Tournament Director Checklist - Please utilize this list before, during, and after an event to ensure that you are completing all USAV event requirements.
  • Policies & Procedures - Review the Florida Region Policies & Procedures page, where you will find the following event-specific resources:

step 2 - apply to be a fl region club or​ an event affiliate
It is NOT necessary to submit both applications. Please read the information carefully to determine which application is best for your organization.

The 2025-2026 Season Club Applications will open in Summer of 2025.
​
  • 2024-2025 Club Application* - If you wish to register your club with the Florida Region of USAV, please complete this application prior to completing the event sanction form. If you do NOT  wish to register a club, please submit the Event Affiliate Application below. More information about club requirements can be found HERE.
  • 2025-2026 Event Affiliate Application* - If you are not currently associated with a Florida Region club OR if your club will not be the main sponsoring organization for your event, please submit this application prior to completing the event sanction form.

*Please note that both of these are annual applications and are valid for the current season only (each USAV season runs from September-August).

Event Sanctioning Guide

Click here to view the ​Event Sanctioning Guide. The Florida Region has implemented a tiered event system to give tournament and event directors more freedom to deliver an event that suits their target market.

Event Sanction application

Use the online forms below to request the approval of a future USAV sanctioned activity or event. 
  • Indoor Event Sanction Request
  • Outdoor Event Sanction Request

Please submit an Event Sanction Application for each event (tournament, scrimmage, clinic, etc.) you are hosting. If you are running a league, please submit one Application for each section of the league (i.e., Summer Beach League, Fall Indoor League, etc.).

Please do not submit your application until your Club Application or Affiliate application has been approved.

Use this online form to request a certificate of insurance for a facility being used to host a USAV sanctioned activity. Please submit a request for each USAV sanctioned event or activity.

​The 2025-2026 COI Request Form will become available in Fall of 2025.

  • ​​Florida Region Incident Report Form - To be filled out at the event by the on-site medical personnel.​
  • Medical Consent Form - To receive medical care at a USA Volleyball sanctioned event or activity in Florida, the completed form will need to be presented to any medical provider before care may be administered.​
  • ​2024-2025 USAV Medical Claim Form - If you wish to file a medical claim, please follow the instructions on this form.

​Florida Region Results Template - Submit this file for any FL Region USAV sanctioned tournament (Level I, II, or III). If your event is run on SportWrench, download the Final Finish Report under the Event Info tab.

USAV National Ranking System Template - Submit this file for any FL Region USAV sanctioned tournament (Level I, II, or III). AES Tournaments do not need to submit this file. If your event is run on SportWrench, download the USAV National Ranking System Export under the Event Info tab.​

  • Indoor Child on the Bench Waiver
  • Regional Uniform Waiver
  • Age Waiver - The Florida Region does not issue age waivers. Please review the USAV Age Definitions, found on the USAV Forms and Information Page.
