Spectator Code of conduct
Spectator Code of Conduct - The Spectator Code of Conduct is required to be posted at all USAV events in the Florida Region.
usav event waivers
  • Indoor Child on the Bench Waiver - This waiver allows a child aged 8 or under in designated bench areas with a rostered adult (who must be the child's parent or legal guardian).
  • Regional Uniform Waiver - This waiver is only valid at Florida Region sanctioned events and may only be valid for one event. 
  • Age Waiver - The Florida Region does not issue age waivers. Please review the USAV Age Definitions, found on the USAV Forms and Information Page.

florida region alcohol policy
​Regional Alcohol Policy - The Florida Region of USA Volleyball prohibits the possession, sale, purchase, and consumption of alcoholic beverages at all sanctioned youth/junior volleyball events. Alcoholic beverages are not permitted to be present or consumed on the premises of any sanctioned/insured space of any venue, including common areas inside the sanctioned/insured area.

Florida region Events

The following policies and procedures will be followed at all events hosted by the Florida Region. Please email [email protected] if you have any questions about these policies.
REGIONAL BID ACCEPTANCE POLICY
Teams receiving a Florida Region bid to the USA Volleyball Girls Junior National Championships (GJNC) are required to accept the bid on-site at the conclusion of the event where the bid is awarded.

Bid acceptance, whether it is an original or trickle down bid, is mandatory unless the offer is made 30 or more days after the conclusion of the Florida USAV Gold Series (for 13u-17u age groups), the ASICS Florida Volleyball Challenge (for 11u-17u age groups) or the Florida Regional 18s Qualifier (for 18s age group only).

BID DIVISION REQUIREMENT - ASICS FL VB CHALLENGE
Any team wishing to compete for a Florida Region bid at the ASICS Florida Volleyball Challenge (AFVC) must compete in at least three (3) USAV Level I ranking events. The deadline for bid eligibility will be as of two weeks prior to the first day of the AFVC.

TEAM CHECK-IN REQUIREMENTS
IMPORTANT: We do NOT offer or accept ON-SITE team check-in.

  • COACHES OR CLUB DIRECTORS MUST complete Online Team Check-In for their teams prior to tournament play. Paper rosters will NOT be accepted.
  • Upon completion of Online Team Check-In, each coach assigned as a "Primary" staff member will receive a unique QR code pass via text & email for entry into the event.
  • There is a MAXIMUM of 4 “Primary” staff members per team. Any additional members will need to purchase passes online for entry into the event.
    • Staff Roles allowed on a roster:
      • ​Head Coach
      • Assistant Coach
      • Club Director
      • Chaperone
  • To gain entry into the venue(s) AT ANY TIME, each Primary staff member will need to present their QR code pass (can be printed or on a mobile device) with their corresponding photo ID at entry.
  • Athletes will need to show their jersey to gain entrance to the venue.
  • If you have any athletes that are 18 years old BEFORE or ​DURING the event, they must have SafeSport training to be eligible to play and for the team roster to be valid for check-in.
 
Teams that do not check-in by the respective time will be dropped from the tournament, be issued a sanction from the Florida Region, and will not be issued a refund for the event.
FORFEITING TEAMS PENALTY
2026 Officiating Fee for Forfeiting Teams* - $TBD per match
  • Referee (R2) - $TBD per match
  • Table Personnel (Scorer & Lib Tracker) - $TBD per match
  • Line Judges - $TBD per match

​*All forfeits must be approved by the Event Director in advance and payment must be remitted at the time of any approval.

SERVICE ANIMAL NOTICE
Service animals are individually trained to do work or perform tasks for persons with disabilities. Service animals are to remain on leash and under the control of their handler at all times. Comfort or emotional support animals are not service animals and are not permitted. Pets are not permitted. Thank you for your compliance with this policy!

2024-2025 USAV Medical Claim Form - If you wish to file a medical claim, please follow the instructions on this form.
Medical Consent Form
Medical Consent Form - To receive medical care at a USA Volleyball sanctioned event or activity in Florida, the completed form will need to be presented to any medical provider before care may be administered.
Medical Release Form
Medical Release Form - This form is no longer required. However, the Florida Region recommends that clubs collect this form from every athlete.
