Instructions: This screen is for players that ACCEPT their invitation to the 2011 Team Florida HP All-Star Camps in Winter Springs and Tampa.  Please be sure you review the Team Florida HP All-Star information (CLICK HERE) before you accept.  When you SUBMIT your information below you are also indicating that you are available to participate (if chosen) in the 2011 USA Volleyball High Performance Championships in Tucson, AZ between Jul 25-30, 2011.  You are also agreeing to comply with all Team Florida HP All-Star rules and guidelines for the duration of your participation in our program.

Once you SUBMIT your information below you will be redirected to a payment screen with information on how to remit your $250 training camp fee. 

IMPORTANT NOTE...ACCEPTANCE is a 3 step process:

Step 2 - Complete the Player Size Information form below.

Step 3 - Remit the $250 Training Camp fee.

All 3 of these steps must be completed by midnight on Sunday, May 1, 2011 for you to secure your position at the respective Team Florida HP All-Star Camp!  On 5/2/11, alternates will be contacted to fill any spot not accepted by the May 8th deadline!

IMPORTANT NOTE: Players are responsible to bring the following items to their camp: shoes, socks, spandex or shorts (black or navy only), knee pads, Team Florida HP tryout t-shirt, club warm-ups, backpack/gym bag, water bottle, medical release form (notarized) and any prescribed medications.  Additional Camp t-shirts will be provided once you arrive.
